What SAGE membership includes…
-
Appointments are available in person for routine care, acute care and chronic management of conditions. Dr. Shadmon is also able to manage many visits with telehealth options and we’d rather you didn’t take a half day off work for a follow-up that can be handled in other ways.
-
We don’t want to make you feel hurried during visits, which is one of the main reasons we built SAGE Direct Primary Care. Here visits are intended to allow enough time to get a full picture, and use time to discuss important risk and benefit discussions, and create individualized plans that you feel comfortable with.
-
A urinary tract infection, a cough that’s turned, a rash you’d like looked at today. You’ll see Dr. Shadmon rather than needing to go to an urgent care or ER, or seeing someone else. And you’ll get your needs met the same day or next day.
-
Questions come up in between visits. You are welcome to reach out and Dr. Shadmon will get back to you in a timely fashion.
-
Blood pressure, cholesterol, diabetes, thyroid, and the ones that arrive together. The work is adjusting things as they change and noticing early when something has, which needs a doctor who saw you last time and remembers what we tried.
-
Screening on schedule, vaccines, and the conversation about what's worth doing at your age and what isn't. Most people in this area already show up for their annual. What's usually missing is the twenty minutes afterwards to talk about what the results actually mean for you.
What patients come to SAGE for
Perimenopause and menopause care
Sleep that stopped working, a memory you don't quite trust, weight that has moved and stayed, and a mood that isn't yours. All of it gets taken seriously here, including the parts you've stopped bothering to mention. Dr. Shadmon prescribes hormone therapy where it's appropriate, and will talk you through what's known, what isn't, and what she'd consider if it were her.
Weight and metabolic health
Dr. Shadmon talks honestly about anti-obesity medications, including GLP-1 medications, and works with you on a personalized plan to tackle the complexity of this chronic medical condition. She medically supervises the weight loss itself and the metabolic health that goes with it.
Preventive care and chronic conditions
We provide full-scope adult primary care, with the follow-through. We monitor chronic conditions such as blood pressure, cholesterol, thyroid and diabetes, and we make sure you are up to date on your annual exams and your age- and risk-based screenings. We pride ourselves on caring for the whole person, not just body parts or systems.
Policies
Enrollment Fee
New members are required to pay a one-time, non-refundable enrollment fee of $99 per person. This fee covers the cost of creating your patient record, setting up your medical account, and other administrative expenses.
Membership Fee
Your monthly membership fee begins at the time of enrollment. After that, your monthly membership fee will be charged on the same date each month. For example, if you enroll on the 12th, you'll be billed on the 12th of every month going forward.
Cancellation Policy
We understand that things happen and plans change. We hope to build lasting relationships with our patients, but if you need to cancel your membership, we ask for 30 days' notice. There are no penalties or cancellation fees. If you decide to rejoin at a later date, you may be asked to pay a new enrollment fee.
Credit Card Surcharge
A 3% processing fee applies to payments made by credit card. There is no additional fee for payments made by debit card, HSA/FSA debit card, or ACH bank transfer. Credit card processing fees are subject to change, and we encourage ACH payments whenever possible to help keep processing costs low.
