Welcome to SAGE Direct Primary Care
We are built on the idea that good medical care starts with a Supportive environment, an Accessible physician, Guided care, and Excellence in every decision.

What is Direct Primary Care?
It's a way of practicing medicine that puts the relationship back at the center of medicine.
The way it works is straightforward. You join SAGE Direct Primary Care for a monthly fee, and that covers your primary care with Dr. Shadmon, which includes your visits, the questions that come up in between, and the follow-up afterward.
You won't be waiting weeks to be seen, and you can book an appointment when you need one.
We don't bill insurance and there are no hidden fees. You'll keep your insurance and use it for what it does well, like specialists, imaging, hospitals and emergencies, and we'll help you use it. We also accept HSA and FSA funds for your membership with us.
It's simpler than it sounds. What you'll notice is that visits stop feeling rushed, and that the person you call is the person who already knows you.
What makes SAGE different?
Longer Appointments
There are no fifteen-minute appointments here. Visits last 30 to 60 minutes, so there's time to get through everything you came in with, and no reason to save something for next time.
The Same Doctor
Dr. Shadmon will know your history, remember what matters, and take the time to understand you. The goal is long-lasting relationships that make personalized care possible!
Direct Access
You can message or call Dr. Shadmon directly with questions between visits, new symptoms, prescription refills, or referrals. Nothing is too small to ask, and there's no call center or waiting for messages to be passed along. She will work to get your needs met in a timely fashion.
No Surprise Bills
Your monthly fee covers all the primary care services at SAGE. There are no copays or deductibles and no surprise or hidden bills. HSA or FSA can also be used for your membership fees.
“There’s a different way to talk to every single person. That’s the art of medicine, and it’s getting to know patients well enough to build trust.”
— Dr. Ella Shadmon
After more than twenty years in one community, Dr. Shadmon has cared for just about every kind of person there is.
