About SAGE Direct Primary Care

A Primary Care practice in South Pasadena, built by a doctor who has dedicated her career to her patients in this community.

A sage is someone known for wisdom, a trusted guide whose advice is respected because it comes from deep knowledge and experience. After more than 20 years in family medicine, Dr. Shadmon strives for that for her patients: someone who knows you, listens closely, and helps you make thoughtful decisions about your health. The name also comes from the sage plant, which has been valued for centuries as a symbol of healing and well-being. Together, these two meanings capture what SAGE DPC stands for: care rooted in wisdom and experience, and focused on healing the whole person.

I am so happy you found me, and welcome to my practice! My relationship to the practice of medicine started in childhood. My father was an eye doctor, the old-school kind, who loved the work and imparted to me the art of medicine. Watching my father treat patients and meet them where they were, building trust and treating everyone as equals, made it so that I couldn't see myself doing anything else.

I trained in Family Medicine at Pomona Valley Hospital, a UCLA program. I found Pasadena during residency in 2000 and never left the area. I spent the next 23 years with the same medical group, practicing traditional insurance-based medicine. Over time the group grew a great deal, and somewhere in that growth, the room to practice the way I wanted, with the same attention to patient relationships, kept getting smaller.

I started SAGE Direct Primary Care in my own community in 2026, with a desire to return to the old-school style of direct access to your doctor, creating a trusting doctor-patient relationship and delivering excellent care.

I have special interests in midlife medicine, especially for perimenopausal and menopausal women, and in obesity medicine, where I have seen amazing results with medically supervised weight loss medications and programs.

In addition to seeing patients, I also spent about 15 years working with this growing medical group on many clinical IT projects, leading multiple changes over those years. This role allowed me to be in the rooms where electronic medical records and patient portals got designed, and I understand better than most why reaching your doctor became so complicated. The system around us just kept getting more complicated, and there was less and less ability to improve the doctor-patient experience.

Outside of being a doctor, I enjoy downtime at home, taking walks with my husband or neighbors, painting by numbers, and reading. I am an empty-nester with two daughters who are on their own journeys. My husband is a wonderful community member who has been involved in countless community events. He is also a travel aficionado, and we enjoy taking trips, seeing new cultures and tasting new foods.
